Add or Modify your info

Adding a New Record

Select 'Add'
Enter your Last name and First name
If you want to select your own password, enter it. If you leave the password blank, a password will be generated for you.

If you know that your information is not on file, enter all of the information that you want to add
If you think your information may already be on file, either enter only your First and Last name, or perform a 'Search' for your info and then use your record number to 'Modify' your info

If you enter too much info for your record, and you already have a record on file, the system may not be able to identify your record. The system will only check records that match ALL of your field data.

Changing your information

Select 'Modify'
Enter your Password
Enter your Last Name or your Record number

(Hint -- Your Record Number may be obtained by searching the database for your information)

A note about the Search routine: When the database is searched, it will only find those records that exactly match ALL of the information that you have entered into the searchable fields. For example, if you enter MUMP but the record has MUMPS on file, the search will be unsuccessful.

Note: Bold fields are required fields. Fields followed by a '*' are searchable

To gain direct access to an entry, enter its record number here.

Password: (case insensitive)
The Password is used for verification purposes. If you want to change or delete your information, you will have to enter the correct password. If you are submiting a new entry, you may leave the Password field empty and the system will generate a password for you.

Name entries: Enter only your last name in the Last Name field. If you have an "identifier" on the end of your name, such as "Sr." or "III", etc., add the identifier to the end of your First Name with a comma, such as "Dan, III"

Comments* (limit to 120 characters):
We suggest using the Comments field to enter relevant info that you feel people would search for, such as what you do, or who you work for, or what groups you belong to, etc.. However, the field can be used for anything that you wish.
